Collapse of foundation for large Pressure test Facility kills four workers.

  • The collapse of the steel structure of the foundation for a large Pressure test Facility at Claxton Engineering Services killed four workers.
  • An investigation by the Health and Safety Executive (HSE) identified serious flaws in the planning, management and monitoring of the project.
  • The workers were employed by Hazegood Construction Ltd, with Encompass Project Management Ltd as the principal contractor and Hazegood also operating as a contractor.
  • As a result of the accident's investigation, Claxton Engineering Services Ltd and Encompass Project Management Ltd plead guilty to regulation 9(1)(a) of the Construction (Design and Management) Regulations 2007 and section 3(1) of the Health and Safety at Work etc Act 1974 (the Act) respectively. Both received various fines.
  • The director of Encompass also pleaded guilty to breaching section 37(1) of the Act and was subsequently sentenced to a seven and a half month custodial sentence, suspended for two years.
  • He was also ordered to complete 200 hours of unpaid community work within 12 months, and also paid prosecution costs of £7,500.
